Case Management Services
Coleman provides case management services to adults with severe and persistent mental illness, including coordinating and participating in the development of an individualized, comprehensive service plan and offering assistance in crisis intervention and stabilization.
Case Managers meet directly with consumers and families to discuss assessing services, including ways to manage basic needs and daily living skills. Case Managers assist clients in encouraging and developing support systems, finding needed community services and eliminating barriers to seeking or maintaining education and employment.
