Q. Where are you located?
A. Coleman Behavioral Health and Coleman Employment Services is located in downtown Canton at the corner of West Tuscarawas and McKinley:

Q. Where do I park?
A. Parking is conveniently located at the office location. The entrance can be found off 400 West Tuscarawas Street. You will need to get a ticket that will be validated when you check in for your appointment or meeting.

Q. How can I change my appointment?
A. You can talk to staff to make changes at the time of your appointment. At other times, you can change appointments by calling 330-438-2400 and speaking with the operator.

Be sure and talk about the cancellation and no show policy during your first appointment or with your service coordinator.

Q. What are some other things that I should know as a customer?
A. Violence against others or property, fighting, disorderly conduct, threatening, intimidating, and use of obscene or abusive language is prohibited on Coleman property.

The use or possession of illegal substances is prohibited on Coleman property. Possession of any weapons, ammunition, explosives or firearms is not permitted on Coleman property.

Q. Will there be a fee for services?
A. Persons receiving services at Coleman Behavioral Health Stark are responsible for paying for the services received. Coleman Behavioral Health staff will assist you in utilizing insurance or enrolling in other programs to subsidize the cost of services when available. We accept Medicare - Medicaid (Caresource, Buckeye & Unison). We do not accept private insurance. Customers that have private insurance can pay full fee if they wish to receive services. All persons receiving services are responsible for providing us with current information on income, insurance or Medicaid eligibility at the time of service. Services that are not eligible for payment by Medicaid, Medicare, insurance or other subsidy programs, including Coleman Access and local ambulance services are your full responsibility.

Coleman Professional Services is a private, non-profit agency providing mental health services to individuals, couples, groups and families residing in Stark County. Through our contract with the Mental Health and Reocvery Services Board of Stark County, we are able to offer subsidized services to those individuals or families unable to pay the total cost of services

Q. How do I contact staff at Coleman?
A. If you need to talk with staff at Coleman, you can use ACCESS after hours, holidays and weekends for non-urgent issues at 1-877-762-3555

You should also ask each staff you are working with about the best way to contact them. Each service differs- some you can call directly and for others you may need to leave a message.

Coleman strives to provide you with ways to access staff in a timely way. Most staff has voice mail and case managers have cell phones for easy access.

Use Crisis Recovery’s 24hr line at 330-452-6000 for all crisis or emergency call after hours, holidays or weekends.

Q. What is a service coordinator?
A. The service coordinator is a professionally trained staff person who is responsible to coordinate your treatment while you are receiving services at Coleman.

They will work with you; your family and/or your guardians to keep your treatment on track and working together while you are in services. Your primary service provider could be a doctor, counselor or case manager and will depend on the services that best meet your needs.

In most cases, you will have a say in who is involved in your treatment and treatment coordination. You will learn more about who is involved with your treatment when you discuss things like - releasing information and confidentiality.

Q. With whom do I speak if I am not happy with services?
A. A staff person will review Coleman’s client rights policy, how you can talk with the client right’s officer, and how you can talk to a client rights advocates outside Coleman. The client rights policy is a separate document that you will have an opportunity to see, talk about and keep for your records.

Coleman also has the policy posted in many waiting rooms and other public places in case you need the information. The client’s right and grievance policy is a formal process that you can use if you have questions or are faced with issues that you have been unable to work out or resolve with your service providers.

Q. What are “outcomes,” and how does Coleman use the information?
A. There are many ways to check out how you are doing and progressing with your treatment. The Ohio Department of Mental Health has a standardized survey tool know as an “outcomes survey”to help you and your service provides see how you are doing in treatment. You will receive more information during your first visit and as you work with staff.

Q. Why do I have to fill out so many forms?
A. Coleman works to keep the paperwork at a minimum for you and your family. In many cases, forms are required for payment, are part of the law, or are requirements of funders. In some cases, paperwork is part of what is needed to provide the best care possible. If you have a question about a form or paperwork, please let a staff person know – Coleman wants you to understand what a particular form or survey is about.

Q. What do I do if I am getting services from other health care agencies or providers; or an agency; or schools?
A. Please inform staff during your appointment time of your involvement with other health care providers, agencies or schools. Coleman strives to provide comprehensive and coordinated services. Knowing what other services you are involved with in the community will help provide coordinated care and avoid duplicating as well as avoiding unnecessary services

Q. What are Coleman’s hours of operation?
A. Our offices are open Monday through Friday from  8:30 a.m. - 5:00 p.m. Coleman observes the following holidays to commemorate significant events and people by closing the major operation of the company.
